The Chernobyl disaster was one the reasons which led to the fall of the Soviet Union. The reasons were 

• design flaws, 
• low quality of equipment, 
• personnel incompetence, 
• “storming” to meet unwarranted deadlines, 
• negligence of safety and security, 
• the state’s misinforming the public and unwillingness to admit the truth.
